Intake forms are found at the bottom of this page. Please print, complete, and bring your forms to your appointment. If you are a distance client, you may e-mail all of your documents to christine@herbchick.com 48 hours prior to your appointment. This will allow for any adjustments or printing challenges that may occur.

Begin a Food Log. You can use the template below or make your own and bring the day of appointment.
Please bring a dedicated notebook to ALL consultations. Here you will record your programs, notes and observations, discoveries and homework for your sessions and any questions you may have for your follow up appointments. This will become a very valuable and necessary resource.
If you are currently taking medications, please speak with your pharmacist about any and all side effects, contraindications, physiological depletions and prescribed purpose. Bring this information to your session.
Take your time. Details are important. Nothing is trivial. If it crosses your mind I want to know about it. Look over your forms and put them down and revist and add to it the next day. Ask a family member or friend if there is something you may have forgotten. The more we have the better.
Don't be limited to boxes and lines. If you feel you need to add a page or so, please do so.
About the Forms:
- These are Google Docs. If you have gmail account and wish to complete on line, download and copy forms and open in Google Workspace .
- Create fields in the designated areas to insert information.
- If you wish to print them out, download as a PDF. When complete, makes sure you use a scanner or the scan option in Notes if you have an Iphone to scan them in. For best results, check to make sure each page is square and level.
